The test reports must be printed in book format per code whereby one copy must stay in the book and the original will be filed on the applicant's file and the second copy shall be handed to the applicant after the test. The abbreviations must be printed on the inside of the front cover of the book and on the reverse side of the copy of the applicant. (Second copy) The test reports must have serial numbers.
